All supervisors must complete this form before supervising any pool session.

I confirm that I have:

[ ] Read and understood the Operating Procedure document.

[ ] Read and understood the Emergency Action Plan (EAP).

[ ] Read and understood the Pool Rules and Terms and Conditions.

[ ] Understood and accept the Supervision Requirements:

• under 8s must have an adult on the water with them, 4 - 7 year olds have a ratio of one adult to two children in the water and under 4s have a ratio of one adult to one child.

• Two adults present at all times.

• Max 1 hour supervision followed by a 20-minute break.

[ ] Hold a valid First Aid qualification (e.g., Emergency First Aid at Work) and am confident in administering CPR and the use of an AED.

[ ] Am physically capable of recovering a casualty from the water.

[ ] Will bring a working mobile phone to every session.
